Phone number changed automatically without my authorization

I ported my number from another provider to Fizz and activated my SIM on my ported number. I received the SMS message to confirm my intentions which I did and everything went smoothly. After just 2 days, I'm no longer receiving phone calls and text messages only to discover that the phone number currently mapped to my SIM is different from my number. How did this happen and how can this be corrected? I haven't changed phone number in over 15 years and I can't afford to lose that number.
