How to I send a Second "Transfer Number" Request?

I signed up with Fizz and got my SIM in the mail. But the first phone transfer failed because I switched SIMs before getting the number transfer confirmation text from my old provider (Public Mobile).

As per the message in my notifications I reactivated my plan with my old provider and can received texts and messages through my old SIM. But I cannot send a second transfer number request because when I do it says..."This information matches the one you already submitted. Please revise and resubmit."

What do I do?
