What phone model are you using? And why do you think you need the PUK code?
Have you just recently activate your Fizz mobile plan? What does the phone display when you insert the Fizz SIM card and turn on your phone?
If your phone is still locked with a previous provider, you will need to contact them to have it unlocked at no charge: https://fizz.ca/en/faq/what-is-an-unlocked-phone
If you want to contact Fizz, you can Live chat with support:
1- Connect to your Fizz account by clicking « Login » in the top right corner at fizz.ca
2- Any ad blockers in your browser should be disabled.
3- Open a help link page from Fizz Solution Hub: https://fizz.ca/en/faq/how-do-i-use-puk-code-to-unblock-my-sim-card
4- The green chat bubble should appear after a few seconds in the bottom right corner.4 -

Please, contact your previous provider.
