Apologies for the encountered issue.
I have verified your modem in my tools and it's status does not seem to be ok. Please unplug the modem from the power outlet and verify if the coaxial cable is tightly screwed in on both ends. After that plug the modem in a different power outlet, without using extender cords or power bars, and test.
If the issue still persists, I suggest to perform a factory reset by pressing the button on modem's back for 30 seconds with a tiny sharp object.
