Usually the support ask to buy a new one and they will refund at the activation that one that you did not received
You can contact Fizz support by chat, from 8 AM to 9 PM everyday:
1- Connect to your Fizz account by clicking « Login » in the top right corner at fizz.ca
2- Click on this link: https://fizz.ca/en/faq/whats-my-rewards-program
3- The green chat bubble should appear after a few seconds in the bottom right corner.7 -

You can order a new one (the 1st one was probably lost or stolen) and when you activate it you can ask for a refund.6

You just need to access an FAQ, scroll through it and after 10-15 seconds the chat bubble will appear in the bottom right corner of the page.
