I can see that everyone provided useful information and you managed to submit the unsubscribe request for your internet plan.
However, I noticed that you didn't receive any details regarding the modem return. I want to inform you that the easiest way to return the modem would be to bring it to a Videotron store in your area. Also, just in case you are not aware of this, I want to mention that you have the option to move your Fizz internet plan to a new address if you would want to bring us over.
