You need to enter it during the plan activation
The referral code must be entered in the field provided at the bottom of the order summary.

If you continue with the sign up process, it should be on the last page before confirming the order. Keep filling out the required information and eventually on one of the final pages where they ask you to confirm everything, you should have the ability to add the referral code.

To add to the previous answer, you can also enter the referral code at the bottom of the SIM order form. Once it is time to activate a plan, the same referral code will already be pre-filled on the form.
Click on the arrow to apply it again just in case you should see "ka-ching" message to let you know the code was entered. 1 -

You can add the referral code at the very end of the subscription process, in the entry titled Referral on the page of the order summary for the very 1st created plan on that account only. Make sure to click on the green button next to it to apply the code! More information on this can be found here:
