I assume your old SIM card has not been activate for a while.
If this is the case, you can use it to activate a new plan.
Is your existing number with another provider? If yes, at time of Fizz activation, you can request your number to be ported over to your new Fizz plan. You will need to provide Fizz with your old provider account and remember that your old provider account must be kept active. Then your old account will automatically close once the port request is complete.
If you're a new Fizz customer and looking for a referral code, please use mine 66KGA when signing-up. We will both be rewarded with a $25 credit once you have paid your second monthly bill.

You can reuse the Fizz SIM, therefore, you can use it again
this is the procedure, if you suspended your plan, otherwise skip the points that do not apply to your case (probably go directly the #4)
Replace your phone and reactivate your suspended plan
To reactivate your Fizz plan:
- Order a new SIM card. Log into your account to order a new SIM card (My plans > Manage > Change SIM card). A new SIM card will be delivered in the following days, depending on the chosen delivery method.
- Acquire a new phone.
- Unblock your plan by sending us a form Report that you found your phone/sim
- Reactivate your Fizz plan on your new SIM card. When you have your new phone and SIM card, it’s time to reactivate your suspended plan on your new SIM card. This process will cancel your old SIM card. Log into your account to change your SIM card (My plans > Manage > Change SIM card).
If you found your Phone/SIM card, you can also reactivate your old SIM card / black list phone.

Hey! On the back of your SIM card, it should say the activation code, when I lost my packaging for the sim card I was worried I wouldn’t have the activation number. But it should be a alphanumeric combination and can be used to active a new plan. Regarding restoring your old account you’d need to get in contact with a fizz representative and they may have a backup or so. Good luck!5
