Hi everyone,
Does anyone know if there is a way to receive a proper bill/receipt for using Fizz, including my name and address? I tried to find them in my account, but the closest thing I could get was the transaction history, which does not help me much as it does not include any personal information.

Fizz is supposed to be used for personal use, therefore, this is the reason.8

I believe that in time, they will provide more detailed ones :-)
I used what they currently provide to submit as an expense to my employer and it has been approved :-) Maybe they used the account number as the confirmation that this is an official invoice...1 -
You could have your Fizz Services agreement including your name and address on it ** under My Plan, Manage plan, Services agreement.6

The receipt shows your phone number and some credit card info.
As others have mention, you can include your "Mobile Service agreement". It shows personal info (name, address and client number).
