Unsuccessful PUK trials

Hi,
My Samsung 6edge requested for my PUK number which I provided but, it was unsuccessful. What next do I do? I'm so confused

    You can look up your PUK number on the Fizz website. Login to your account and then go to My plans -> Manage my plan -> Unblock my SIM card. Your PUK code is 8 digits. Make sure that your phone is actually asking for the PUK code and not something else, such as a phone-specific passcode. You would only get a request for the PUK if you have turned on PIN entry for your SIM card at the some point in the past and entered an incorrect PIN three times. The default PIN, Fizz SIMs come with, is 1234.
