Either you pay by speed (bandwidth mb/s) like home internet, or by amount of data (gigs) like cell plans. Or some combination of both.
Reason is pretty simple, at home a desktop or laptop is super fast and for gaming especially: latency kills. So companies monetize speed.
Whereas mobile data is slow in comparison and since speed is the problem, no provider will celebrate slow connections... they all claim fast service, though not true. So the only thing to monetize is the overall data used.

I cannot think of one internet service provider that does not offer unlimited plans. They do however drop the speed significantly after a certain amount of data has been used.
Pay As You Go plans are offered, which can be limited by the amount of money that is in your account - when you run out of money you can buy more data. If you have an unlimited amount of money you can buy an unlimited amount of data.
I do not understand your second question. Please elaborate.
