I think that your modem must be changed. For that, you must chat with an agent directly 24/7 using the live chat function. To contact them on the live chat you have to go to any of the FAQs and click the (?) in the lower right corner of the screen. Once you do that you will have to click on the (...) three dots which represent the live chat function.

I suggest you to contact Fizz's support directly and ask them to change your modem. It's pretty clear with your pictures that the modem has to be changed.
It looks like the cable that goes from the wall outlet to the back of the modem, or the plug that connect to the wall outlet!
If this is the case, try to carefully cut the bent tip straighten the wire and re-connect.
