Modem login page is blank

My router has stopped being able to get an IP address from the modem. I connected my PC directly to the modem and I was able to connect to the internet, so I was going to dig around in the modem settings to figure out what was going wrong. When I connect to 192.168.0.1, I end up on this mostly blank page. It doesn't ask for any login information. Any help?

    I finally just did a factory reset on the modem. Problem solved.
