I did not see any place to put the referral code. I only ordered sim card. I did not activate it

Mohamed, the referral code is added when you order a service ( phone and /or internet), not the SIM card. When you make your order choice it will be one of the last bits of info you provide.13

It is not mandatory to enter the referral code when you order the SIM card.
You must enter your friend's referral code when you are signing-up for (activating) your first plan. It has to be entered on the screen where you see the total price including the taxes.

Remember that the bonus referral credit of $40 is available if you activate before October 15th. After that promo, the credit goes back to $25.

Mine is there too. To be transparent, both parties will receive the bonus credit once you have paid your second bill.13 -

The field for referral code should be there in page you presented.
Did you stop the processus of activation in any way? If you stop the process of activation and restart later, the system consider that the account is not new (the referral code can be applied only to a brand new account). The same is happen if you or go back and forth on web pages, when you pass the second time on the same page is like the account is not new anymore, so you can't use a referral code.
In my opinion, DO NOT CONTINUE till you contact ASAP Fizz, possible they may help you and your friend to benefit from referral (is 40$ for each of you till 15 October).
If you don't know how to contact them, the easy way to do it is by facebook here: https://www.facebook.com/fizzca/ they answer as fast as they are available, or by browser; daily from 8AM to 9 PM (after you login to your account) goto https://fizz.ca/en/faq/im-unable-log-my-fizz-account-what-do-i-do scroll page down and after few seconds the green chat bubble should appear.

It's when you activate your sim card and when you see the price of your plan without the taxes you can put your referral code.6 -
You add your referral code at the very end of your first month payment, when you activate your account. You have a place to enter the code AND you have to click on the green box to the right for it to register.6
