if you used the PUK provided and does not work It seem like your phone is lock by another provider. Can you verify that?
Did you used the PUK from:
1- fizz account,
2 - In the panel under your name in "My Plans",
3 - At right panel you see your name click "Manage Plan",
4 - You should see "Unblock SIM card "
5 - Click "Unblock SIM card" and you'll "Reveal your PUK code", there is a light grey on/off radio button right underneath it.5 -

It seems your phone isn't asking for PUK code. Instead it's asking for PIN code to unlock phone.
Was this phone purchased through a cell provider or used for a specific provider?
If yes, please contact them to get the unlock code. Provide them the IMEI of your phone, they are required by law to give you the unlock code at no charge.3 -
I believe that your phone is SIM lock, you need to ask your previous providers to unlock it
