What address do I send my modem to?

I just deactivated my internet service plan, and now I have to send back my modem.
However, I don't really know what to do with the code bar provided. Also, I don't know the address I must send my modem to. HELP

Hi! You'll find detailed instructions here: https://fizz.ca/en/faq/how-do-i-return-my-wi-fi-modem. Essentially, place the modem in any box that can be taped shut and tape the bar code/label to the top of the box, then drop it off at any Canada Post and keep the receipt
