Where to get invoice or receipt with name and phone number

I need a receipt that shows my name and phone number. I tried the option under transaction history but it doesnt provide enough information.

However, there you can find only an invoice with your customer number, you need to download your service agreement to prove that it is under your name

 Your Fizz Transaction history lists every transaction you have made to pay for each mobile plan on your account. Transactions you may see here include monthly plan payments, deposits to your Wallet or Add-On purchases.

To see a list of all your Fizz transactions:

1. Go to My Plans in your Fizz account.
2. If you have more than one plan, choose the plan you wish to see transactions for.
3. Scroll to the bottom of the usage summary and click on Transaction history.

Go to your account online,
Click My plan
then after: Manage your plan
Click also: historic of transactions
Choose one billing invoice of the month you need.
Click Print (the invoice) at the end of this invoice
All the current specific information of your plan is there.
