Can I transfer a number from an account with a different name?

I want to transfer my daughter to Fizz, and put the account in her name. Currently the number I want to port over is on a different carrier, but it's under my name, not hers.
When I create an account on Fizz, should I put it in her name or mine? Note...I already have a seperate account in my name on Fizz.
Note: I have to re-ask the question as it automatically closed when I accepted an answer.

You need to have the IEMI of the phone.

Based on other threads I read
It looks like people seem to have more success with Account#

ok.. so the name on the source account doesn't need to match the name on the destination account?

I've transfered two times my number using IMEI without any issues.

spoke to customer service once more
asked them again
they have confirmed that source and destination name do NOT need to match
as long as you have all the details
either
A- phone number and account number
B- phone number and IMEI
