Why on my account I'm Level 2, but on the forum I'm Apprentice 3/5?

You get points for your activity in the Community Hub and the ranking (Apprentice 3/5) is for Community Hub. If you look at the leaderboard & activity in the right hand corner, you can click on "my account", click the star and you can see how many points you need to move to the next level. I believe this is only used for the Community Hub and I don't know if there is something tangible that you will receive based on this ranking (only to let others know how helpful you are on the Community).
The account level 2 has a different point system which includes your activity in the Community Hub, but also includes points for your activities with Fizz in general (paying your bills, gifting data, etc). Because the Apptentice 3/5 only means that you've been a client of Fizz for 3 billing cycle, while Level 2 is related to your engagement with the community.

Sorry @Hugo L., but the Apprentice 3/5 does not mean you've been a client for 3 billing cycles. I am 3/5 and definitely have not been a client of Fizz for 3 billing cycles. It really is about your activity in the Community forums.6

Kent, it's not about your participation in the forums either, since I've seen people ask one question, answer none, and yet still be on apprentice 2/5. I agree it's not about billing cycles either though, because I'm 3/5 but on my 4th billing cycle. It's a mystery.7

Take a look at the ? in the LeaderBoard & Activity. It says:
In order to energize its community, Fizz wishes to reward the actions that each of its members carries out on its site You will be able to accomplish goals generating points, take up challenges, win trophies, follow the members of the community and gain notoriety. 1. Please connect on Fizz 2. Open your Dashboard 3. Check your goals and complete them to earn points 4. Follow the members you are interested in and expand your network The team Fizz
In that same box, you see activity and the points being awarded.
If I look at My account in Leaderboard & Activity, and choose Rank (trophy) you see an Obj. If you hit that objective, I think you get to the next level. It looks like each of us has different goals (you can click around different users) and you can see all the people with their ranks and what it would take to get to the next level. This is where there is a mystery of how the objectives are set. But the points are definitely coming from your activity.
@Allison W., it looks like you need 37.6k pts to move on to the next level 4/5, but I only need 6.5k pts.

What? No. 37.6k points is how many I've earned since the beginning in the community. It's not connected to the apprentice ranking, but it contributes to your leveling up in your fizz account.
This screen shot shows the dates I got each level--it's clearly also not a monthly anniversary thing.

You're right. I just looked at the day stats and it matches the points in the activity list. My bad on assuming that the Obj. was correlated to the pts showing.
It's definately a mystery on how many points is needed to move to the next level. Looking at the different profiles, I have way fewer points than Allison, but we have the same Apprentice 3/5.

Only Level on your profile is relevant for the reward program.
Apprentice Level, to my knowledge, it just tells the community how many cycles you have been active, since you have created an account.
I don't think you don't need to have made a payment, you just need to be active the forum.
For every Apprentice level you reach, you get 50 points on your reward program profile.
However, it has been confirm to me by specialize reward team, that the whole reward program is under review. So we should see a lot of change in a very near future.

It's not really cycles you've been active either as far as I can tell. If you look at the screen shot I linked above, I got 1/5 on 11/07, 2/5 on 12/01 and 3/5 on 12/26. Not a month apart and none of them correspond to my payment dates.3
