If you log to fizz.ca, then click My Plans, Manage Plan, you shoud find an option that says Unsubscribe.

Alternatively, you can just let your current plan ends on its own. Fizz is prepaid, you will not get any reimbursement. Sorry to see u leave!
p.s. If you want to port your phone# to other provider, you need to port-out while your plan is active.
