I have a 500 Meg upgrade and perks on 20 gigs, I was thinking of lower my daughters plans to get the cheaper price with less data and put the upgrade and perk on her account , my question is will the upgrade and perk stick on her account forever?

No the perks expire usually 30 days after the activation and you have x days to activate (it is shown on the perks)

At most, you can have 3 slots, and is not so easy to reach at least level 5 to get the 3 bonus Fizz loyalty program: My Rewards
