Go to My Plans in your Fizz account.
Scroll to the Plan you wish to change.
Click on Manage and choose Change SIM.
Select Order a SIM card.
Select your reason for ordering a new SIM card and click Continue.
Add your shipping address. This is the address where we will ship your SIM card. It may or may not be the same as your home address.
Choose your shipping method.
Select a payment method and click Submit order to pay for your SIM card.
... When you receive your SIM card, go back to Change SIM and select Activate your SIM to replace it.

Sorry about this loss of your phone;
You can but a sim card from Fizz directly to a (Dépanneur) Couche-Tard near you if you are living in Montreal.
Or, you can order a new sim card into your Fizz account and wait 4,5 days to receive it and activate it with a new phone.10 -
