Nop, just the modem will do. I believe you need to give the modem and the power cord. You can choose the box you like. Amazon box can do

I think you are not obligated to keep it. However I kept my box because it is brand new and the modem fits perfectly inside. When the time comes, you can ask Fizz support whether they prefer the modem to be returned in the original box or if any box will do.
Note that the home internet contract does mention that we are responsible for damages to the modem.6 -

You can use any box to return the modem back, my friend use an old box and was delivered to Fizz without any problem!5
