You need to contact Fizz for this issue.
To initiate a chat session with Fizz open a help page ( example https://fizz.ca/en/faq/im-unable-log-my-fizz-account-what-do-i-do ). You'll see a bubble chat at bottom right in a few seconds1 -
You need to order another sim card. Probably this one was lost or stolen.
Contact the support to let them know that you haven't received the sim. They will tell you to order another sim, then they will reimburse you for the first one.
If you live in a building, the delivery guys stick the envelope to the entrance door because they don't have access to enter the building like Canada Post. So next time check thoroughly if the envelope is there when you receive a delivery notification.1
