You can't manually change the APN settings on an iPhone. Instead, make sure to download and install the latest iOS and then you'll be prompted to update the carrier settings. If you want to know the date your credit card was charged, you can click "My Plans" --> "Transaction History". Future monthly charges will occur the day before the day of the month you activated your plan (see David P.'s reply).

It depends on the cause of the loss of signal. In the case of the recent power outage in Ottawa - it was due to physical damage to mobile towers and power stations, which of course takes a lot longer to repair.
