Thanks for the help Ihssan. The problem was caused by the phone I'm using, which needed was requesting the PUK but was in fact requesting the master unlock code. I found the solution here :…

I got it 4-5 year ago from Virgin Mobile. I'm guessing it's locked by them but isn't the PUK code suppose to be doing just that, unlocking it from this provider ? Also when entering the pin the first three time, was I supposed to enter the Virgin mobile one ? or just enter PUK since it's disabled on fizz sim cards?
